Managed Care Organization Authorizations

Managed Care is a health care delivery system organized to manage cost, utilization, and quality. Medicaid managed care provides for the delivery of Medicaid health benefits and additional services through contracted arrangements between state Medicaid agencies and managed care organizations (MCOs) that accept a set per member per month (capitation) payment for these services.

  • Daily Living Skills
    -We provide real life skills to our participants as needed to help them be successful in the community.  This includes but is not limited to balancing a checkbook, budgeting, laundry, bus training, meal prep, or anything that a person needs to live and work independently.
  • Housing Counseling
    -We assist participants in identifying barriers and work to find housing whether it be renting or owning their own home within their financial means.  Follow along services may be provided to assist participants maintaining successful community living as well as supporting landlords with issues that may arise.
  • Transportation
    -We provide transportation to and from work sites with authorization
  • Community Prevocational Service
    A 7-week course for individuals who may need training in soft skills, personal skills and professional skills. This course is person-centered and community focused. Along with skills training, participants will gain exposure and experience in our community through worksite visits, volunteer opportunities and guest speakers from area businesses or professionals.
  • Enclaves
    Area businesses contract with Embark to provide a small group of participants with job coach support on site to conduct services like janitorial, mowing/yard cleanup, snow removal, retail, production/assembly work or food service. You are paid by Embark and not considered an employee of that business.
  • Community Integrated Support
    Once you accept a position and DVR services are not being used, Embark can provide long term on-the-job support for you and your employer. For example, Supported Employment Outcome Based (SEOB) services through Inclusa or other long term supports through additional funding sources.
